Job summary

TEEMA is seeking a Licensed Physical Therapist to evaluate and treat geriatric patients in a non-weekend, Monday–Friday schedule. You will assess mobility, plan exercise programs, and deliver a comprehensive rehabilitation plan focused on maintaining independence.

You will document data, develop measurable care plans, educate patients and families, supervise aides, and collaborate with an interdisciplinary team to ensure high-quality care. Licensure and geriatric experience preferred.

Qualifications

• Licensed PT with active license in good standing
• Experience with geriatric population preferred
• Ability to document and communicate treatment plans clearly

Responsibilities

• Collect and document patient data, identify problems, and set priorities
• Develop care plans with measurable goals and interdisciplinary collaboration
• Provide and document prescribed treatments, ensuring adherence to standards
• Educate patients and families on rehabilitation needs and interventions
• Supervise aides and staff, ensuring cost-effective rehabilitation practice
• Participate in interdisciplinary team meetings and care planning
• Effective teamwork and communication with the team
• Other duties as assigned
