Job summary

Sayville Rehabilitation & Nursing Center is seeking an experienced Physical Therapist to assess patients needing rehabilitation and to develop individualized treatment plans. You will lead direct care, document progress, and educate patients and families on therapy techniques.

The role requires a PT license, graduation from an accredited program, and coordination with therapy staff and other disciplines to ensure optimal patient outcomes in a long-term care setting.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Exceptional patient service skills and clinical excellence.
• Report to work on time and adhere to scheduled hours.
• Perform all other duties assigned by Director of Rehabilitation.
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred.

Responsibilities

• Assess referred patients and provide direct care to determine rehabilitation needs.
• Establish and administer treatment programs with goals based on patient capacity.
• Document patient condition and educate patients and families on therapy methods.
• Supervise and coordinate with Physical Therapy Assistants.
• Collaborate with other disciplines to ensure comprehensive patient care.
• Maintain professional conduct and timely documentation.
