Job summary

FOX Rehabilitation in York, PA is seeking licensed Physical Therapists to deliver one-on-one care in private residences and senior living communities within our Geriatric House Calls model. You’ll help older adults improve mobility, independence, and quality of life with autonomous scheduling and meaningful patient relationships.

Clinicians may incorporate telehealth and other virtual care to extend access, support more patients, and maintain high-quality outcomes.

Qualifications

• Licensed PT in Pennsylvania or eligible to obtain licensure.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, memory care).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
