FOX Rehabilitation is seeking a licensed Physical Therapist in Nashua, NH to deliver one-on-one geriatric care in private homes or senior living communities. You’ll assess mobility, develop plans, and guide patients toward greater independence through our Geriatric House Calls model.

The role emphasizes autonomy, flexible scheduling, and telehealth options when appropriate, with collaboration from an interdisciplinary team and ongoing professional growth within a clinician‑driven culture.

Competenze

• Valid Physical Therapy license in the state(s) of practice or eligible
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Mansioni

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, mobility, and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate to extend access
