Job summary

FOX Rehabilitation is seeking licensed Physical Therapists in Lowell, MA to provide one-on-one care in homes or senior living communities under the Geriatric House Calls model.

You’ll evaluate, plan, and deliver evidence-based therapy while maintaining a flexible schedule and the option for telehealth to extend access and continuity of care. MA licensure and CPR certification are required, with private residences and communities as settings.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth/virtual care when appropriate to extend access and flexibility.
