FOX Rehabilitation seeks licensed Physical Therapists to deliver in-home and senior living care in Chatham, NJ. You will provide one-on-one PT to older adults, assess and plan care, and use telehealth when suitable. The role emphasizes mobility, independence, and aging in place.

Formación

• Valid Physical Therapy license in the state(s) of practice
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsabilidades

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, longevity, mobility and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
