Job summary

PACE Southeast Michigan is seeking a Physical Therapist to evaluate and implement individualized treatment plans with an interdisciplinary team. The role focuses on improving mobility, restoring function, maintaining safety, and reducing pain for participants aging in place.

Requirements include a Physical Therapy degree and Michigan licensure, plus at least one year of experience with frail or elderly populations.

Qualifications

• Must hold a Physical Therapy degree (Bachelor, Master, or Doctoral).
• Active State of Michigan licensure.
• Minimum 1 year of experience with frail/elderly population.
• Ability to instruct and demonstrate exercises, provide lift assistance, and manage mobility devices.

Responsibilities

• Coordinate rehabilitation services with the Interdisciplinary Team.
• Perform assessments and provide treatment sessions.
• Implement care plans and document care in medical records.
• Order/issue Durable Medical Equipment (DME).
• Maintain accurate clinical records and participate in professional development.
