Job summary

PACE in Westland, MI seeks a Physical Therapist to evaluate and implement individualized plans to improve mobility and reduce pain. Onsite role collaborating with the interdisciplinary team to provide comprehensive care for participants aging safely in their homes.

The PT will perform assessments, deliver treatment sessions, and document care while ensuring safety and quality. Michigan licensure and a year of geriatric experience are required.

Qualifications

  • Physical Therapy degree (Bachelor, Master, or Doctoral).
  • Active State of Michigan licensure.
  • Minimum one year of experience with a frail or elderly population.
  • Ability to instruct and demonstrate exercises, provide lift assistance, and manage mobility devices.

Responsibilities

  • Coordinate rehabilitation services with the Interdisciplinary Team for participants in PACE Southeast Michigan.
  • Perform assessments, deliver treatment sessions, implement care plans, and manage DME orders.
  • Document all care in accordance with department standards and submit records timely.
  • Maintain accurate clinical records and utilization data for the rehabilitation department.
  • Participate in professional associations, educational programs, and in-service education.
  • May supervise support staff and delegate tasks to PTA and Rehab Technician.
  • Complete home safety and facility assessments; document prior to care conferences.
  • Contribute to site assignments, program development, and quality improvement projects.
