Majestic Care of West Allen is seeking a Physical Therapist to evaluate, plan, and implement personalized therapy for SNF residents, focusing on mobility, strength, balance, and functional independence. You will work with an interdisciplinary team to deliver resident-centered care, guided by HIPAA and regulatory best practices.

Qualifications

• Geriatric rehabilitation knowledge for SNF residents.
• Experience collaborating with interdisciplinary care teams.
• Strong clinical documentation and regulatory awareness.

Responsibilities

• Conduct initial and ongoing evaluations of residents’ PT needs (mobility, strength, balance).
• Develop and implement individualized treatment plans based on findings and goals.
• Provide PT interventions: therapeutic exercises, gait/balance training, manual therapy.
• Educate residents, families, and caregivers on plans, home exercises, safety strategies.
• Collaborate with nursing, occupational therapy, and speech therapy teams.
