Job summary

Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings. In this on-site, patient-centered role, you will implement PT treatment plans focused on mobility, safety, and quality of life.

Qualifications

• PA-licensed Certified Physical Therapist Assistant (CPTA) required.
• Experience in geriatric skilled nursing or long-term care preferred.
• Strong communication and documentation skills in EMR.

Responsibilities

• Provide PTA services to seniors in SNF/long-term care.
• Implement and adjust PT treatment plans.
• Document progress accurately in EMR and educate patients/caregivers.
• Collaborate with an interdisciplinary care team.
