Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team on-site. You will provide individualized therapy to seniors in skilled nursing and long-term care, implementing PT plans to improve mobility, function, safety and quality of life.

You will collaborate with an interdisciplinary team, document accurately in EMR, and educate patients and caregivers.

Formación

• Licensed PTA with active credential in the state where practicing.
• Strong ability to implement evidence-based PT plans for seniors.
• Experience in skilled nursing or long-term care settings desirable.

Responsabilidades

• Implement PT treatment plans focused on mobility, function and safety.
• Lead therapeutic exercise, gait and balance training, and functional activities.
• Ensure accurate EMR documentation and patient education for caregivers.
