Job summary

Element Care seeks a Personal Care Specialist to support participants in the PACE program at the ADH and in the community under a licensed nurse. You will assist with personal care and meals, monitor vital signs, and accompany participants to appointments.

Preferred candidates hold an HHA certification and at least one year of geriatric care experience, with a commitment to safety, dignity, and team collaboration.

Qualifications

• HHA certification or equivalent.
• Minimum one year of geriatric care experience.
• Ability to pass a fit test and wear respirator safely.

Responsibilities

• Assist participants with personal care at site and in the community.
• Assist with meals, including 1:1 feedings, and follow dietary restrictions.
• Monitor vital signs and assist with transfers.
• Escort participants to medical appointments as needed.
• Restock supplies and maintain clean exam rooms and common areas.
• Maintain CPR training and adhere to safety procedures.
