• Manage a panel of patients with terminal illness focusing on symptom management.
• Develop individualized plans to improve quality of life.
• Lead and collaborate with the interdisciplinary team.
• Order and interpret labs and tests; prescribe treatments.
• Coordinate referrals to specialty services as needed.
• Educate patients and families about care options.
• Respond to palliative care consults within 48 hours.
