Job summary

Hospice Savannah is seeking a qualified LPN to join the PACE clinic team in Savannah, GA. The role delivers general nursing care under supervision of PACE providers, performs nursing interventions, and triages participant medical conditions according to standing orders.

You will reinforce nursing education for participants and caregivers, schedule referrals to medical specialists, and collaborate with an interdisciplinary team to ensure safe, patient-centered care.

Qualifications

• Current Georgia LPN license in good standing.
• Minimum 1 year experience with frail/elderly population or completion of PACE training.
• Competency in venipuncture and blood specimen collection.

Responsibilities

• Provide general nursing care under supervision of PACE providers.
• Execute nursing interventions and triage participant medical conditions per protocols.
• Schedule participant appointments for PACE Clinic referrals to specialists.
• Educate participants, caregivers, and PACE staff per established teaching plans.
