Job summary

Hospice Savannah seeks a licensed Physical Therapist to deliver comprehensive PT services to PACE participants in Savannah, GA. You will assess, plan, intervene, educate, and monitor to support mobility and safety, collaborating with the IDT to tailor care plans.

Travel within the service area is required; the role is full-time, non-exempt, with standard hours and potential home visits to support participants' needs and care quality.

Qualifications

• Master’s degree in Physical Therapy (Required)
• Valid driver’s license and reliable transportation
• One year of experience working with a frail or elderly population, or the ability to receive appropriate training upon hire (Required)
• Must not have been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under Title XX (Required)
• Must not have been excluded from participating in Medicare or Medicaid programs (Required)
• Must not have been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse (Required)
