Job summary

Aston Group is seeking a compassionate Licensed Occupational Therapist to join our Skilled Nursing Facility rehab team in Lakeland, FL. You will evaluate residents, develop individualized treatment plans, and provide OT services to promote independence and safety.

Qualified candidates hold a Master’s or Doctorate in OT, Florida licensure, and preferably experience in geriatric rehabilitation and Medicare guidelines. Strong communication and documentation skills are essential.

Qualifications

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.
• Current OT license in good standing in Florida.
• Skilled nursing or long‑term care experience preferred.
• Knowledge of geriatric rehabilitation and Medicare regulations.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide therapy focused on ADLs, functional mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.
• Complete accurate and timely documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care planning and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.
