Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to join our collaborative team serving seniors in skilled nursing and post-acute care settings. In this patient-centered role, you will evaluate and treat residents, develop individualized plans of care, and help patients maximize safety, independence, and quality of life.

You will collaborate closely with an interdisciplinary team, document in EMR, and educate patients and families.

Qualifications

• Licensed OT valid in Illinois with a focus on geriatrics.
• Experience evaluating and treating seniors in SNF or post-acute care.
• Ability to educate patients and families and document accurately in EMR.
