Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to join our collaborative team serving primarily seniors in skilled nursing and post-acute care settings. In this patient-centered role, you will evaluate and treat residents, develop individualized, outcome-focused plans of care, and help patients maximize safety, independence, and quality of life.

You will collaborate closely with an interdisciplinary team, document in EMR, and educate patients and families.

Qualifications

• Active OT license in Indiana or eligibility.
• Experience caring for seniors in SNF or post-acute settings.
• Ability to develop individualized, outcome-focused care plans.

Responsibilities

• Evaluate residents and administer evidence-based treatments.
• Develop and implement plans of care to maximize safety and independence.
• Educate patients and families and document progress in EMR.
