Job summary

Therapy Management Corporation (TMC) seeks a licensed Occupational Therapist to empower adult and geriatric patients through compassionate, patient-centered therapy in a collaborative clinical setting. You will focus on improving independence, mobility, safety, and quality of life while delivering high-quality, evidence-based care.

In this role you will evaluate patients, develop individualized treatment plans, and coordinate with interdisciplinary teams to ensure optimal outcomes and continuity

Qualifications

• Licensed OT or license eligible in the state of practice.
• Graduate of an accredited Occupational Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life.
• Deliver care through ADL training, therapeutic activities, and adaptive techniques.
• Help patients improve daily tasks such as dressing, bathing, grooming, and meal preparation.
• Partner with interdisciplinary teams and nursing staff to support outcomes and continuity of care.
• Track patient progress and document clinical notes, adjusting treatment plans as needed.
