Job summary

Flagship Therapy in Bountiful, UT is seeking a full‑time Occupational Therapist for our South Davis in‑house rehab program. You will screen and evaluate patients with cognitive and ADL challenges, develop care plans, and provide rehabilitative, skilled treatment in accordance with regulatory standards.

Responsibilities

• Screens and evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment interventions.
• Administers procedures as part of the rehabilitation plan (ADL retraining, therapeutic activities, exercises, modalities, assistive devices).
• Assesses progress and adjusts treatments in the clinical record.
• Evaluates treatment outcomes and refers as appropriate.
• Provides consultation to patients, families, caregivers and other providers.
• Communicates progress, barriers, and plans with supervisor and health team.
• Generates comprehensive discharge summaries per regulatory requirements.
