Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans following standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Assist with daily activities requiring standing/walking; assist with transfers and patient positioning.
• Administer rehabilitation procedures including manual techniques, ambulation, exercises, modalities, and wound care.
• Provide counseling to patients, families, and caregivers regarding physical disorders.
• Generate discharge summaries in compliance with regulatory requirements.
