Job summary

BAYADA Home Health Care is seeking a full-time Occupational Therapist to deliver rehabilitative services to geriatric patients in Assisted Living and Independent Living communities across Mansfield, Attleboro, Easton, Norton, Westwood and Dedham. You will travel to client homes, evaluate function, and develop personalized plans with a focus on safe, independent living.

The role emphasizes collaboration with a multidisciplinary team, ongoing documentation, and patient education.

Qualifications

• Massachusetts OT license required.
• Minimum one year of OT experience, preferably in home health.
• Experience delivering therapy for geriatric clients.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of functional status, endurance and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and plan interventions.
• Evaluate home environment for hazards and independence barriers.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Revise care plan with nursing multidisciplinary team as appropriate.
• Educate patients and families in rehabilitative care and safety.
• Document observations and interventions using a touchscreen tablet.
