Job summary

Horsham Center for Jewish Life is seeking an Occupational Therapist (OT) to join our compassionate therapy team in Horsham, PA. You will evaluate residents, develop individualized OT plans, and provide skilled services as ordered by physicians.

You will set short- and long-term goals, monitor progress, document outcomes, and collaborate with nursing and other professionals to maximize residents' independence and quality of life.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current OT license in good standing required.
• Geriatric population experience preferred.
• Strong communication, documentation, and teamwork skills.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide skilled occupational therapy services based on physician orders
• Establish appropriate short- and long-term goals
• Implement and modify treatment plans based on resident progress and clinical needs
• Monitor resident response to treatment and document progress
• Participate in resident assessments and care plan development
• Collaborate with nursing, therapy, and other interdisciplinary team members
• Provide resident-centered care focused on maximizing independence and quality of life
