Job summary

Terrace Health and Rehab in Gainesville, FL, is seeking a dedicated Occupational Therapist (OT) to evaluate residents and deliver skilled OT services in a SNF setting. You will develop personalized plans to improve activities of daily living and promote safety and independence, working closely with an interdisciplinary team.

Responsibilities include comprehensive assessments, ongoing plan modification, resident and family education, and treatment documentation in accordance with facility

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Florida OT license or eligibility.
• CPR/BLS certification if required by facility policy.
• Experience in skilled nursing or post-acute settings preferred.
• Knowledge of OT principles, evaluation methods, and documentation.

Responsibilities

• Conduct comprehensive OT evaluations and develop individualized care plans.
• Provide skilled OT interventions to improve ADLs and functional independence.
• Collaborate with nursing, physicians, families, and therapy staff.
• Educate residents and caregivers on treatment plans and safety.
