Job summary

Blue Stone Therapy in Perry, IA is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric residents in a SNF and post-acute setting. You will implement treatment plans under the supervision of a licensed OT to improve ADLs, mobility, strength, and cognition.

The ideal candidate is a graduate of an accredited COTA program with an active license (or eligible) and strong communication, organization, and clinical reasoning skills.

Qualifications

• This role requires a graduate from an accredited Occupational Therapy Assistant program.
• Active OT COTA license or eligibility for licensure in the applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehab is preferred.
• Strong communication, organization, and clinical reasoning for patient-centered care.

Responsibilities

• Provide skilled OT treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional outcomes.
• Treat adult and geriatric patients in SNF and rehab settings.
• Monitor patient progress and communicate with supervising OT.
• Maintain accurate documentation to support quality care and reimbursement.
• Collaborate with interdisciplinary team to support positive patient outcomes.
