Job summary

Blue Stone Therapy in Cedar Rapids, IA is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients at Heritage Speciatly Care.

Under the supervision of a licensed Occupational Therapist, you’ll implement plans designed to improve ADLs, functional mobility, strength, cognition, and independence in SNF and post-acute rehab settings.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Occupational Therapy Assistant license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Provide skilled occupational therapy treatment in accordance with established plans of care
• Implement individualized treatment programs focused on functional and measurable outcomes
• Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings
• Monitor patient response to treatment and communicate progress with the supervising Occupational Therapist
• Maintain timely, accurate documentation to support quality care and reimbursement standards
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
