Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care to develop care plans.
• Deliver skilled, medically necessary treatment interventions.
• Manage physical demands including standing, walking, lifting, transferring, and repositioning.
• Administer interventions such as manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Provide counseling to patients, families, and other providers on physical disorders.
• Generate discharge summaries in compliance with regulatory and clinical requirements.
