Job summary

BoldAge PACE is seeking an Occupational Therapist to join our interdisciplinary team to help seniors live independently at home with dignity. You will assess needs, plan and deliver therapy, and collaborate with families and caregivers to maintain participant well‑being.

Responsibilities include assessments, adaptive equipment coordination, treatment planning, and OT interventions across the PACE center, clinic, and participants’ homes.

Qualifications

• Advanced degree in Occupational Therapy from an accredited school.
• Current license to practice Occupational Therapy in the state.
• 2 years of experience on an interdisciplinary team preferable.

Responsibilities

• Conduct initial and periodic assessments of participants’ functional abilities and home environment.
• Coordinate adaptive equipment, home modifications, and therapy services.
• Develop, implement, and revise individualized treatment plans with IDT collaboration.
• Deliver OT interventions across PACE center, clinic, participants’ homes and contracted facilities.
• Educate participants and caregivers on equipment use and safety and ADL programs.
• Maintain timely documentation per regulatory and organizational standards.
• Participate in daily IDT meetings and care planning.
• Supervise COTAs and provide mentorship.
• Train staff in safe OT practices and equipment use.

Education

Advanced degree in Occupational Therapy

Current license to practice Occupational Therapy in the state
