Job summary

Therapy Management Corporation (TMC) is seeking a licensed Occupational Therapist to empower adult and geriatric patients to improve independence and quality of life in a collaborative clinical environment.

You will provide patient-centered OT services, develop individualized care plans, and work with interdisciplinary teams to support meaningful outcomes, safety, and functional skills in daily activities for older adults.

Qualifications

• Licensed OT in the practicing state or license eligible.
• Graduated from an accredited Occupational Therapist program.
• Experience in skilled nursing, long-term care, or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life in older adults.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence and safety.
• Deliver care through ADL training, therapeutic activities, and adaptive techniques.
• Help patients perform daily tasks such as dressing, bathing, grooming, and meal preparation.
• Collaborate with interdisciplinary teams, caregivers, and nursing staff to support outcomes.
• Track patient progress and document clinical notes and treatment plans.
