Job summary

Therapy Management Corporation (TMC) is seeking an Occupational Therapist to deliver patient-centered therapy to adults and geriatric populations. You will evaluate patients, develop individualized plans, and collaborate with interdisciplinary teams to improve independence, mobility, safety and quality of life.

In a supportive, clinician-driven environment, you will work with skilled nursing facilities and senior living communities to ensure consistent, high-quality care and meaningful patient

Qualifications

• Licensed Occupational Therapist (OT) or license eligible in the state of practice.
• Graduate of an accredited Occupational Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life.
• Deliver care through therapeutic activities, ADL training, and adaptive techniques.
• Help patients improve their ability to perform daily tasks such as dressing, bathing, grooming, and meal preparation.
• Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals.
