Job summary

Fusion Physical Therapy Partners is seeking a dedicated Occupational Therapist to serve residents in assisted living facilities in Bedford, NH. You will evaluate and treat geriatric patients, create individualized care plans, and collaborate with a diverse team to enhance daily living activities and mobility.

You will lead wellness initiatives, educate families, and document care using EMR, supporting high-quality, patient-centered outcomes across locations in New England.

Qualifications

• Graduate of an accredited occupational therapy program.
• Active or pending state license to practice occupational therapy.
• Passion for delivering quality, evidence based patient care.
• Superior customer service and interpersonal communication skills.
• Experience or interest in geriatrics, with skills in adaptive equipment and ADL training.
• Strong communication and interpersonal skills to work effectively with patients, families, and colleagues.

Responsibilities

• Conduct patient evaluations to assess functional abilities, daily living skills, and cognitive performance
• Develop and implement individualized care plans focusing on activities of daily living (ADLs), functional mobility, and other goals that matter to your patients
• Lead wellness initiatives and group fitness classes to encourage healthy habits and social engagement among patients
• Collaborate with an interdisciplinary team including physical therapists, speech therapists, and nursing staff to support optimal patient outcomes and quality care
• Educate patients and families about strategies to improve independence and better quality of life
• Document patient care services in compliance with state and facility standards using EMR
• Deliver care at one or more assisted living locations
