Job summary

Engage Senior Therapy is hiring an Occupational Therapist to work with geriatric populations in Bedford, NH. You will evaluate patients, create individualized care plans, and lead wellness initiatives across assisted living facilities. Collaboration with physical therapy and nursing teams is essential.

You will educate families and patients on independence strategies, document care in EMR, and provide services across multiple locations, contributing to patient quality of life.

Qualifications

• Graduate of an accredited occupational therapy program.
• Active or pending state OT license.
• Passion for delivering quality geriatric care and collaboration.

Responsibilities

• Conduct patient evaluations to assess functional abilities, daily living skills, and cognitive performance.
• Develop and implement individualized care plans focusing on ADLs and mobility.
• Lead wellness initiatives and group fitness classes.
• Collaborate with an interdisciplinary team to support outcomes and quality care.
• Educate patients and families about strategies to improve independence.
• Document patient care services in EMR compliant with state/facility standards.
• Deliver care at one or more assisted living locations.
