Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role will evaluate residents, design individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist with current licensure.
• Experience in skilled nursing or post-acute facilities preferred.
• Ability to document effectively in an EMR system.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver evidence-based OT interventions to improve ADLs, mobility, cognition and safety.
• Document progress in EMR and communicate with interdisciplinary team.
• Educate families and caregivers about care plans and safety.
