Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Proven ability to assess residents and determine OT needs.
• Ability to develop individualized treatment plans for seniors.
• Experience delivering evidence-based interventions to improve ADLs, mobility, cognition and safety.
