Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifikationen

• Licensed OT with current license.
• Experience with seniors in SNF or post-acute settings preferred.
• Ability to evaluate residents and develop individualized treatment plans.
• Proficient documentation in EMR and collaboration with interdisciplinary teams.
