Job summary

The Staff Pad, partnering with a physician-led healthcare organization, seeks a Geriatric Nurse Specialist (LPN or RN) in Boston to support about 1,600 Medicare patients across independent living, assisted living, and skilled nursing facilities.

You will collaborate with physicians, advanced practice providers, caregivers, and community partners to deliver coordinated, person-centered preventive and chronic care, focusing on aging well and improving quality of life for older adults.

Qualifications

Licensed LPN or RN with an active MA license.
10+ years of experience caring for geriatric patients.
Strong communication and ability to work within an interdisciplinary team.

Responsibilities

Provide clinical care and preventive services for Medicare patients across multiple settings.
Coordinate follow-up care including referrals and ongoing services after wellness visits.
Advance care planning discussions and patient education on medications and risks.
Conduct comprehensive geriatric assessments covering ADLs/IADLs, cognition, mobility, and home safety.
Lead care coordination for high-risk older adults and facilitate transitions between care settings.
