Job summary

Total Life, Inc. is seeking a Telehealth NP to deliver longevity assessments and ongoing geriatric care for adults 65+. You’ll perform functional, cognitive, and medication reviews, and coordinate care with our geriatrician CMO.

This is a 100% remote role with full training and robust clinical support. Ideal candidates have an active CA NP license, geriatric focus, DEA registration, Medicare documentation experience, and comfort with telehealth and EHRs.

Qualifications

• Active, unrestricted NP license in California.
• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus.
• Active DEA registration.
• Experience with Medicare documentation and E/M coding.
• Comfort with telehealth and EHR documentation.
• Reliable high-speed internet and a private, HIPAA-compliant workspace.

Responsibilities

• Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk
• Provide medical follow-up visits for chronic disease management and ongoing care
• Perform cognitive screening and staging; elevate for further assessment where indicated
• Prescribe and manage GLP-1 therapy for appropriate patients
• Conduct advance care planning conversations
• Document in our EHR to support accurate Medicare billing

We provide behavioral health, geriatric care, and longevity services to adults 65+, billed through Medicare, Railroad Medicare, and major commercial insurers. Our network includes 200+ credentialed providers serving patients across 49 states.

The role

You’ll deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth. This includes comprehensive geriatric assessment, cognitive screening, medication review, advance care planning, and management of chronic conditions. You’ll work alongside our CMO, a board-certified geriatrician, with full training and clinical support provided.

• Familiarity with cognitive screening instruments (Mini-Cog, MoCA)
