Job summary

Locumly is seeking a dedicated Nurse Practitioner for a permanent primary care opportunity focused on geriatric patients in a North Carolina setting. This role emphasizes comprehensive, patient-centered care within an interdisciplinary team, including home visits and discharge planning support.

The position requires board certification, active licensure in NC, DEA/CDS/BLS credentials, and 5–7 years of geriatric NP experience, with visa sponsorship not available.

Qualifications

• Board Certified NP with AANP or ANCC certification.
• Active North Carolina NP license.
• DEA registration, CDS certification, and BLS certification.
• 5–7 years NP clinical experience, geriatrics/long-term care preferred.
• Authorized to work permanently in the U.S. without visa sponsorship.

Responsibilities

• Deliver comprehensive primary medical care to frail and elderly patients in collaboration with program physicians.
• Conduct initial, ongoing, and acute clinical assessments.
• Develop, implement, and manage individualized care plans within a multidisciplinary team.
• Evaluate and manage acute illnesses and chronic conditions.
• Conduct home visits to ensure continuity of care.
• Support discharge planning for hospital and long-term care transitions.
• Participate in an on-call rotation for continuous clinical support.
• Maintain accurate clinical documentation per program standards.
