LIFE Geisinger South Wilkes-Barre is seeking a Nurse Practitioner to provide comprehensive medical and supportive services to an adult patient population aged 55 and older in a team-based care setting.

The role emphasizes coordinated care across acute visits, post-hospitalization follow-ups, and annual assessments with 12-16 patients per day and 30-minute appointment blocks to ensure thoughtful, thorough care. The position operates in a clinical environment with an interdisciplinary support team.

Formación

• Licensed Nurse Practitioner in good standing.
• Experience with chronic disease management and acute visits is preferred.
• Ability to collaborate with an interdisciplinary team.

Responsabilidades

• Assesses and establishes presumptive diagnosis and implements treatment plans.
• Prescribes, adjusts or discontinues medications as appropriate.
• Performs procedures within the NP scope of practice.
• Communicates with collaborating physician, patient, PCP and referring staff.
• Provides health education to patients and families in understandable language.
