Job summary

TEEMA is seeking a Nurse Practitioner to deliver direct and indirect patient care in an outpatient geriatric clinic, under the supervision of the Medical Director. You will conduct health assessments, manage care plans, and coordinate patient treatments across settings.

The role requires a Master’s in NP, 3–5 years nursing experience, and ability to operate within CMS-approved protocols; state licensure and immunizations are required. Strong EMR and team collaboration skills are essential.

Qualifications

• Master's degree in clinical nursing from an accredited NP program.
• 3-5 years of nursing experience, NP experience preferred.
• Must meet state competency standards and CMS approvals.

Responsibilities

• Take health histories and perform physical exams; educate patients.
• Order, interpret, and follow up on lab tests and results.
• Manage medications and treatments per physician-approved protocols.
• Coordinate daily patient care under supervision of the attending physician.
• Refer patients to specialists with PCP approval.
• Document changes in the EMR and communicate with the team.
• Participate in on-call schedule and staff meetings.
• Assist in planning 24/7 care with the IDT.
