Job summary

TEEMA seeks a Nurse Practitioner to provide direct and indirect care in outpatient geriatrics under the supervision of the Medical Director/PCP. You will conduct health assessments, coordinate patient care, and collaborate with staff across settings.

Responsibilities include history taking, exams, order interpretation, EMR documentation, and education for patients and families. Requires NP master’s degree and 3–5 years of nursing experience, with geriatric focus preferred.

Qualifications

• Master's degree in clinical nursing from an accredited Nurse Practitioner program.
• 3-5 years of nursing experience, with Nurse Practitioner experience preferred.
• Ability to practice within the scope of authority and meet CMS-approved competencies.
• Experience with geriatrics and ability to coordinate care under physician supervision.

Responsibilities

• Conduct health histories, physical exams, and patient teaching; order and interpret lab tests.
• Perform selected procedures and document care in the EMR; initiate emergencies as needed.
• Manage medication orders and diagnostic studies under physician-approved protocols.
• Assess and coordinate patient care daily under the supervising physician.
• Refer patients to other physicians and specialists with PCP approval.
• Act as liaison with healthcare professionals; educate and support staff, patients, families.
• Participate in provider call schedule and attend staff meetings.
• Document patient changes in medical records and communicate updates to team.
• Evaluate and treat patients in homes, assisted living, nursing homes, and hospitals.
• Precept medical students and residents during clinic rotation.
• Discuss DNR options and complete related documentation with physician guidance.
