Job summary

TEEMA is seeking a Nurse Practitioner to provide direct and indirect care under Medical Director supervision. You will perform health assessments, manage medications, and coordinate patient care across settings including homes and clinics.

Ideal candidates have a Master's in clinical nursing and 3–5 years of nursing experience, with strong EMR skills and experience with elderly populations. Outpatient clinic role with on-site work and on-call hours.

Qualifications

• Master's degree in clinical nursing from an accredited program.
• 3–5 years of nursing experience; NP experience preferred.
• Eligible to practice within state authorities and CMS standards.
• Experience with geriatric populations preferred; training provided.

Responsibilities

• Conduct health histories and physical exams; perform teaching.
• Order and interpret lab tests; follow up on results.
• Manage medications and diagnostic studies per protocols.
• Coordinate patient care and document in EMR daily.
• Refer to specialists with PCP approval; provide family education.
• Attend staff meetings and participate in provider calls.
• Precept medical students and residents during rotations.
• Discuss DNR options and complete necessary documentation.
• Evaluate and treat patients in homes, clinics, hospitals.
