Position Overview:

A Geriatric Nurse Practitioner (FNP) in skilled nursing facilities plays a critical role in the clinical management of geriatric patients in various nursing home facilities. This includes admitting patients, follow-up, and discharging patients from rehab and long-term custodial patients. This position requires close collaboration with the geriatric physicians and the facility’s directors of nursing.

• Provide care to all nursing facility patients, including rehab and custodial patients.
• Performing physical exams and creating a treatment plan.
• Recording patients' medical histories and symptoms.
• Ordering, administering, and analyzing diagnostic tests.
• Monitoring and operating medical equipment.
• Administering medicine and other treatments.
• Detecting changes in patients' health and modifying treatment plans as needed.
• Communicate with patient and family members all care plans and any updates.
• Develop patient care plans and collaborate with nursing staff and patients’ families.
• Organize and provide quality improvement and teaching services to nursing staff within client SNFs.
• Work with a multi-disciplinary team of primary care physicians and other allied clinicians, providing compassionate and high-quality care within acute and post-acute settings.

Education:

• GNP National Certification
• DEA License and IL Control Substance License (preferrable but not required)

Work Experience:

• Clinical background in adult (ANP), geriatric (AGNP), or family specialties.
• Preferred experience with SNFs and an understanding of post-acute care services.

Physical Requirements:

• Ability to walk and stand for long periods of time.
