Job summary

Inspira LIFE program seeks a Nurse Practitioner to provide primary medical coverage for seniors in a team-based environment. You will conduct comprehensive assessments, develop and implement care plans with the interdisciplinary team, and participate in nursing facility visits and discharge planning.

Applicants should hold an NJ NP license with prescriptive authority, CDS/DEA, and have geriatric experience; Spanish speaking is preferred.

Qualifications

• Master's degree in nursing from an accredited program.
• Current NJ RN and APN licenses with prescriptive authority.
• CDS and DEA certification.
• Minimum 2 years of geriatrics experience; Spanish speaking preferred.

Responsibilities

• Provide primary medical coverage for Inspira LIFE participants.
• Perform initial and semi-annual assessments and develop plans of care with the IDT.
• Integrate primary care treatment plans into the overall plan of care.
• Attend to participants in nursing facilities and participate in discharge planning.
• Maintain documentation, participate in on-call rotations, and educate staff.
