Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide medical services to residents in long-term care facilities in Ohio. You will deliver primary and preventative care, manage acute changes, and collaborate with an interdisciplinary team to optimize patient outcomes.

Responsibilities include prescribing medications per OH law, ordering tests, coordinating with nursing home staff, and supporting care plans with primary care physicians.

Qualifications

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and the care plan.
• Collaborate with primary care physician.
