Qualifications

• Nursing degree from an accredited university and graduate of a certified or accredited Nurse Practitioner Program; or Master’s degree in Clinical Nursing and five years nursing experience.
• Must hold a current license as an Advanced Practice Registered Nurse (APRN).
• Preferred: Five to seven years of nursing experience, with three years of experience as a Nurse Practitioner.

Responsibilities

• Collaborate with PACE Interdisciplinary Team to prepare and update Care Plans.
• Conduct comprehensive history and physical exams for new participants.
• Manage participants' medical care for various issues.
• Document changes in a participant's condition in a timely manner.
• Serve as the provider on-call for on-call nurses during evenings and weekends.
