Qualifications

• 6+ months experience as a Nurse Practitioner or Physician's Assistant

Responsibilities

• Provide collaborative primary care to Longevity Health Plan panel in nursing home settings
• Develop and deliver individualized care plans and advanced care planning
• Attend nursing facility care team meetings; communicate with families
• Deliver care in the appropriate setting, in-place when possible
• Document member health status, functional status and diagnoses clearly
• Coordinate with PCPs, families, and telehealth for member changes
• Maintain patient-focused, high-quality care and teamwork
