Job summary

Pine Park Health is seeking a Full-Time Nurse Practitioner to deliver onsite primary care to older adults in East Bay Senior Living Communities (Oakland, Walnut Creek, Concord). This role emphasizes a smaller patient panel with complex care, contributing to a value-based care model and collaborating with a multi-disciplinary team to ensure high-quality patient care.

You will work in a hybrid environment with travel across the East Bay, using tech-enabled tools to support care delivery and care

Qualifications

• Geriatrics experience preferred.
• Ability to work in fast-paced, changing environments.
• Willingness to travel across East Bay San Francisco area.
• Strong collaboration with multicdisciplinary team.

Responsibilities

• Provide onsite primary care services to older adults in Senior Living Communities.
• Develop and execute detailed person-centered care plans for primary care patients.
• Collaborate with Medical Director to address clinical guidelines and policies.
• Conduct monthly Chronic Care Management visits and document CCM.
• Lead regular clinical team huddles and provide direction.
• Coordinate with senior living management to implement Pine Park care plans.
