Job summary

Pine Park Health, Inc. is seeking a full-time Nurse Practitioner to deliver primary care to older adults in senior living communities across the North Bay.

You’ll manage a smaller panel, develop personalized care plans, and collaborate with RNs, MAs, and care navigators to provide coordinated, proactive care. This field-based role requires an active California NP license with furnishing, a DEA registration, and a passion for geriatrics.

Qualifications

• Active, unrestricted California NP license with Furnishing Number.
• Active, unrestricted DEA registration (Schedules II–V).
• Interest in medically complex older adults and preventive primary care.
• Willingness to provide onsite care across the North Bay and drive between communities.
• Comfort with tech-enabled care tools.
• Geriatrics experience preferred (AGNP-C).
• Value-based care experience and home-based, mobile practice exposure.

Responsibilities

• Provide comprehensive primary care to older adults in senior living communities and build ongoing patient relationships.
• Develop and manage individualized care plans reflecting medical needs, goals, and preferences.
• Deliver monthly CCM visits and complete associated documentation.
• Collaborate with RNs, MAs, care navigators, and community staff to coordinate care.
• Lead regular clinical huddles to review needs and coordinate follow-up.
• Partner with community leadership to implement care plans in living environments.
• Collaborate with Medical Director to improve workflows and policies.

Are you a Nurse Practitioner who wants to build meaningful relationships with patients and practice medicine with the time and support to do it well? Pine Park Health is hiring a full-time Nurse Practitioner to provide primary care to older adults living in senior living communities across the North Bay. You’ll manage a smaller panel of complex patients, develop personalized care plans, and work closely with RNs, MAs, and care navigators to deliver coordinated, proactive care — with more time to focus on your patients and less time spent navigating the traditional clinic model.

How This Role Works

This is a field-based primary care role serving older adults in senior living communities across Novato, Santa Rosa, and Marin County. You’ll provide care onsite at partner communities, with travel between locations as needed. You’ll have around patient and community needs, with mileage reimbursement for work-related travel. All providers work from home on Fridays, creating dedicated time for documentation, care coordination, and professional development.

Compensation & Benefits

• Total target cash:$152,000-$195,000, inclusive of base salary and target bonus

What You’ll Do

• Deliver monthly Chronic Care Management (CCM) visits and complete the associated documentation
• Collaborate with RNs, MAs, care navigators, and community staff to coordinate care and support continuity between visits
• Lead regular clinical huddles to review patient needs, coordinate follow-up, and support effective team-based care
• Partner with community leadership and care staff to implement care plans in a way that works in each patient's living environment
• Collaborate with the Medical Director to improve clinical workflows, care standards, and policies, including responses to emerging diseases
