Job summary

Pine Park Health Management Inc. in Santa Rosa, CA, seeks a Part-Time Nurse Practitioner to deliver care in senior living communities within the San market.

This role supports a smaller patient panel on a value-based care model, collaborating with a multidisciplinary team to provide high-quality patient care. • Ability to work with tech-enabled care delivery tools (e.g.,project management, 'checklist' management, remote monitoring, etc.) used together to improve patientcareand team function
• Flexible work schedule with travel within the Santa Rosa area. This role involves driving between partner communities as needed

The Opportunity:

Pine Park Health is looking for a Part-Time Nurse Practitioner (2-3 days weekly) responsible for delivering care in our San market (Santa Rosa area). If you are excited about the opportunity to work with a smaller patient panel of complex care patients, contribute to a value-based care model, and work with a motivated multi-disciplinary team to provide high-quality patient care you have come to the right place. The base salary range for this role is $145,000 to $165,000 (at FTE 1.0) plus generous bonus, equity, and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data.
